Complete the GENERAL INFORMATION SECTION completely.  The PTCAS ID# will be obtained from doing the PTCAS application.

The VERIFICATION OF HOURS OF EXPERIENCE section of your PTCAS application provides you instructions on how your supervising licensed physical therapist verifies your observation experience with PTCAS.  For CSU Sacramento, you must complete a minimum of 100 hours of observed/worked/volunteered experience in at least two physical therapy settings (e.g. acute, rehab, SNF, outpatient orthopedics, pediatrics, geriatric, etc.) under the supervision of a licensed physical therapist. A minimum of twenty-five (25) of those hours must be performed at a single setting.

Filling out the PREREQUISITE COURSES ARTICULATION SUMMARY section

Before filling out Prerequisite Courses Articulation Summary, it will help to have handy a MPT Prerequisite Course Worksheet, to determine if you have completed at least 10 of 14 required prerequisite courses.

For each course, use the drop down lists provided of courses that have been accepted to fulfill the requirements.  For each prerequisite course completed at a college or university other than CSU, Sacramento that have not been previously approved, complete a Request for Substitution of Prerequisite Course form found on our website at http://www.hhs.csus.edu/pt/. You must submit substitution forms by December 1, 2009. 

Provide the term, year, units completed, and grade so that we can verify these on your official transcripts.  Remember that prerequisite courses must have been completed at an accredited college or university with a minimum grade of “C” or better.   If you are taking a course currently, please put “IP” [for “in progress”] in the Grade column.  If you plan to take the course in spring or summer, please put “P” [for plan] in the Grade column. 

The columns to the right prompt you to declare how many times (if any) you have taken a course previously to fulfill the prerequisite requirement.  If you have not taken a course before, simply enter “0” in the Times Repeated box.  If you took the course one time previously, you should mark “1” in the Times Repeated box.  We use the higher course grade in computing your prerequisite GPA.  If you have taken a course three times at ANY COLLEGE/UNIVERSITY, you must mark “2” in the Times Repeated box, and your course grade will be based upon the average of the three grades you received.  TAKING A COURSE MORE THAN THREE TIMES TO FULFILL A SINGLE PREREQUISITE REQUIREMENT DISQUALIFIES AN INDIVIDUAL FROM APPLYING TO OUR PHYSICAL THERAPY PROGRAM. Transcripts from ALL Colleges/Universities must be submitted. 

Anatomy, Physiology, Kinesiology, Exercise Physiology, Pathophysiology and Pharmacology must have been completed within the last ten (10) years at the time of enrollment to the MPT program.

The Additional Background Criteria section

Please check all of the boxes in this section of the supplemental application that pertain to you.  If you check a box,  please write a brief narrative in the space provided.  For Bilingual language competency,  note that there is a distinction between language proficiency and language fluency.  You will need to submit documentation to verify foreign language fluency.   Please do not omit information if you feel unsure about your qualifications; explain your uncertainty in the narrative.  The admissions committee will make final rulings on awarding points based upon your narrative.

The ADVANCED ACADEMIC COURSES section gives you an opportunity to list courses above and beyond the basic required prerequisites.  You should select any courses from the drop-down lists that have been approved as meeting the requirement. No single course may be listed as serving to fulfill more than one required or additional (advanced) prerequisite course requirement.  For example, if you listed a course in the PREREQUISITE COURSES ARTICULATION SUMMARY section, that same course cannot be listed again in this section (In other words no “double dipping”).

Be sure to complete the APPLICATION CHECKLIST section. 

This is where you verify that you understand that 3 separate applications are needed to complete the whole application process to the MPT program at Sac State.   Remember that this application is only one of three that you need to complete before your application is finished.  You must also complete Physical Therapist Centralized Application Service (PTCAS) online application and the Office of Graduate Studies, Graduate Application.  If you are an International Applicant, you also have a fourth application to complete.  Please see the “How to Apply” section of our webpage for detailed instructions.

Recommendation Form(s): Three (3) recommendation forms must be submitted with the PTCAS online application. Two (2) recommendation forms need to be submitted by physical therapists licensed in the United States who have worked with the applicant in a physical therapy setting,  One (1) recommendation form must be submitted from a college-level professor who has been an instructor of the applicant. Forms can be downloaded from the PTCAS website.

It is highly recommended that you print out the VERIFICATION of SUBMISSION screen that you see after you successfully submit your application.

SELECTION PROCEDURES
All application materials are reviewed by the Admissions Committee to rank applicants. Points are assigned for the prerequisite coursework GPA, recommendation forms from two (2) licensed physical therapist and one (1) college level professor, the GRE writing (analytic) score and additional background criteria. The 60-80 highest ranked applicants will be invited for a panel interview. The top thirty-two (32) applicants along with alternates will be notified of their status within two weeks of the final interviews.

Please note: Applications will not be considered if the prerequisite course GPA requirement is not equal to or greater than 3.0 and supporting documentation is not received by the January 15, 2010 deadline.

How to Apply

MPT Program Overview

Applicants should be aware that all physical therapist professional
education programs in the US are subject to accreditation "standards that
assure quality and continuous improvement in the entry-level preparation of
physical therapists, and reflect the evolving nature of education, research,
and practice." While the Education Section of APTA recognizes that there
are proprietary organizations that rank physical therapist education
programs, the profession encourages prospective students to provide their
own comparisons through careful examination of information on program and APTA websites, and visits to the campuses to talk with enrolled students and program faculty.
