Application Requirements
to apply for Fall 2010
enrollment consideration
An applicant must:
- have received (or will receive by the end of the Summer 2010 semester) a baccalaureate degree from an accredited college or university;
- have completed ten (10) of the fourteen (14) prerequisite courses by the January 15, 2010 deadline. The four (4) remaining courses must be completed in the Spring 2010 and/or Summer 2010 semester (a maximum of two of these courses may be completed during the summer);
- have a minimum of 2.5 GPA for the last 60 semester units of your baccalaureate degree, for CSU’s Office of Graduate Studies’ Graduate Application;
- have a minimum of 3.0 GPA in a the prerequisite coursework with grades of C or better in each course is required for the Department of Physical Therapy’s MPT Supplemental Application. To ensure academic coursework will fulfill our requirements, check with the CSU prerequisite course descriptions, and, if your course is not listed as an acceptable course substitute, complete and submit a Request for Course Substitution Form at any time prior to December 1, 2009 for the Admissions Committee to review.;
- submit three (3) separate application forms that are required to apply for the Master of Physical Therapy program.
- Physical Therapist Centralized Application Service (PTCAS) online application ( apply online at http://www.ptcas.org/) due January 15, 2010
- CSU’s Office of Graduate Studies’ Graduate Application (apply online through CSU Mentor http://www.csumentor.edu/) due November 30, 2009
- CSU Sacramento’s the Department of Physical Therapy, MPT Supplemental Application due January 15, 2010
- Physical Therapist Centralized Application Service (PTCAS) online application ( apply online at http://www.ptcas.org/) due January 15, 2010
- have accumulated and documented a minimum 100 total hours of observed/worked/volunteered experience in at least two physical therapy settings (inpatient, outpatient, home, SNF, etc.) under the direct supervision of a licensed physical therapist [twenty-five (25) is the minimum number of hours that can be worked at a facility for that facility to be used for application purposes];
- submit two (2) Recommendation Forms from U.S. licensed physical therapists and one (1) from a college level professor with your PTCAS Application.
- submit Graduate Record Examination (GRE) scores (verbal, quantitative, and analytical writing) by January 15, 2010 [School Code: 4671 and Department Code: 0619].
- applicants who do not possess a bachelor's degree from a post secondary institution where English is the principal language must demonstrate competence in English by scoring a minimum of 550 (213 if computer-based) on the Test of English as a Foreign Language (TOEFL). Submit those scores with your PTCAS Application and CSU Graduate Application
- submit a letter from the director of your previous physical therapy program designating your academic standing (if applicable).
