NCLEX-RN Exam

The following applies to ALL STUDENTS planning to apply for their initial RN license by examination after completion of the current semester. As you go through these instructions, you can click on the links to go to the forms needed to complete the application process.

Please note: If you previously applied to take the exam, but did not take it, you will need to re-submit all forms with updated information. Forms submitted in previous semesters will not be automatically carried forward.


Procedures for applying to take the NCLEX-RN Exam

Step #1: Form’s that need to be submitted to the Board of Registered Nursing (BRN) (complete and submit no later than 45 days prior to the end of the semester). If you have any questions regarding this portion of the application process, please contact the BRN. (www.rn.ca.gov)

    • Complete Application by Licensure by Examination form
    • Pay appropriate fees
    • Live Scan form: Have live scan completed at the qualified agency: Local Police/Sheriff Department or the Department of Justice
    • Confirmation: Self Addressed envelope with a first class stamp affixed
    • A recent (1) 2”x2” passport-type photograph
    • Interim Permit (if applicable)
    • Any additional forms as applicable (see general info and check-off list).

Step#2: Form’s that need to be turned into the Division of Nursing to Nancy Beers no later then November 1st for the fall semester, April 1st for the spring semester and July 1st for summer session.

    • BRN Request for Transcript (this form will be held at the Division of Nursing until your official university transcript is received at the end of the semester. Both will be forwarded to the BRN. Please complete Part A and the Division of Nursing will complete Part B. You can download this form the BRN’s Home Page under Application for Licensure by Examination.
    • Notification of Intent to take the NCLEX-RN Exam: Please indicate whether you will be taking the exam as a Graduate* of the program or as a Non-Graduate, LVN 30 Unit Option or 2nd Bachelor. (*Graduate/Non-Graduate status determines reciprocity in other states). This form is used to complete your Individual Candidate Roster which the BRN will use to determine your eligibility to apply for licensure.
    • Candidate Licensure Requirement worksheet is sent to the BRN to show that you have completed the required prerequisite courses. If all of your pre-requisites were completed before entering the program, it is only necessary for you to complete the top portion of the form (name, social security number, and telephone number) and to sign and date your form. The Division of Nursing will complete the portion of the form requesting for your pre-requisites, course #, title, grade, completion date, and school. If you completed any of these courses, i.e., Societal Cultural, while attending the nursing program and especially if taken at another college or university, you must submit a COPY (original not required) of your transcript(s) or grade report(s) to the Division of Nursing, attention, Nancy Beers. This is not related to the official CSUS transcript required at the end of the semester.

Step#3 Complete the University Request for Transcript Form and turn into Lassen Hall no later then November 1st for the fall semester, April 1st for the spring semester and July 1st for the summer session.

    • The cost for a university transcript is $4.00. If planning to take the NCLEX-RN Exam as a graduate, it is important to check the box to delay the processing of your transcript request for the posting of your degree. If planning to take the NCLEX-RN Exam as a non-grad, all you to need to do is check the box to the delay the processing of your transcript for your current semester grades.
